TERMS OF REFERENCE
Position: Nurse Manager in Home Care Center
Term: Full time
Location: Yerevan, Armenia
Project: Integrated Community-Based Home Care Service Provision and Healthy Ageing Project
Project Duration: 2023-2027
Start date: ASAP
Job responsibilities:
Management
· Coordinating the Community Integrated Home Care Service system in Yerevan, including overseeing the work of staff and volunteers.
· Developing and managing a database for the beneficiaries
· Provision and accounting care equipment and supplies to staff in close collaboration with the ARCS Head Quarter
· Preparing monthly reports and analyses regarding the activities conducted at the home care centers
· Conducting daily monitoring of the hygiene and epidemiological situation at the Community Integrated Home Care Centers
· Conducting weekly team meetings with the home care center staff, discussing the cases, and developing and implementing appropriate action plans and schedules
Nursing Activities
· Initial and periodic assessment of individual nursing issues
· Collaborating with family doctors and nurses to plan and develop individualized care plans
· Planning and monitoring daily home care and assistance for the beneficiaries
· Providing home care in special situations
· Regular evaluation of the results of care activities carried out with the beneficiaries
· Direct supervision of the work of nurses at the care centers
· Direct supervision of the work of care specialists at the centers
· Regular evaluation of the professional knowledge and skills of nurses and care specialists and developing and implementing plans for improvement.
Collaboration
· Establishing working relationships with all relevant community institutions and fostering stable cooperative partnerships
· Raising awareness regarding the services provided within the framework of the Community Integrated Home Care and Active Aging model.
Other Responsibilities
· Carrying out special assignments from program managers and supervisors
· Participating in training sessions, workshops, meetings, and other events organized within the program
· Sharing the acquired knowledge with the work teams
· Actively contributing to the development of materials related to healthcare and social care issues
· Willingness to learn and take initiative
Required Qualifications
· Medical medium professional education with at least 3 years of experience
· Professional management experience in the healthcare sector (will be considered as an advantage)
· Availability of continuous professional development certificates
· Excellent leadership skills, ability to work in a team, and share knowledge with the team
· Ability to quickly adapt to emergency situations and reorganize work accordingly
· Fluent in spoken and written Armenian
· Proficiency in Microsoft Office (Word, Excel) programs
· Knowledge of foreign languages will be considered an advantage
· Possession of a driving license will be considered an advantage
